I/We the undersigned authorize and
request that Jim & Wendy Howard take possession of and disperse the cremated
remains of ________________________________________________________ (full name
of deceased) by scattering them from their aircraft in accordance with all
applicable Federal, State and local laws.
I/We understand that once the remains are scattered they are unrecoverable and
agree that Jim & Wendy Howard have completed their part of this agreement.
I/We hold harmless and indemnify Jim & Wendy Howard from any and all loss,
damage, liability or causes of action, including attorney fees, in connection
with the disposition or the identification of the cremated remains of the
deceased.
The remains container/s will be disposed of by Jim & Wendy Howard.
The obligation of Jim & Wendy Howard shall be limited to the disposition of
the cremated remains as directed herein.
I/We agree that Jim & Wendy Howard are not responsible for any loss or
damage occurring during transport by the US Postal Service, UPS, FedEx or other
carrier chosen by the undersigned.
I/We certify that I/We have the full legal right to authorize the disposition of
the remains of the above-named Deceased.
